Background Investigation

Who:

All Partners and USDA Employees.

What:

Background Investigations are required for all persons that are using USDA equipment or connected to the USDA network or who come in contact with paper copies of information with sensitive data.

When:

Partner’s Background Investigations must be submitted or completed within 7 days of reporting to duty.

How:

Step 1 - District Conservationist follows procedures from TN Bulletin 110-09-3.

Step 2 - Download applicable fillable forms with instructions.

The following forms and brochures require Acrobat Reader.

e-QIP Invite Form (PDF; 57 KB)

SF-85 (PDF; 335 KB) – Questionnaire for Non-Sensitive Positions

SF-86A (PDF; 166 KB) - Continuation Sheet for Questionnaires

SF-85P   (PDF; 520 KB) - Questionnaire for Public Trust Positions - For Contract (ACES) Employees only - in place of SF-85.

OF-612 (PDF; 93 KB) – General Information - Optional Application for Federal Employment - OF-612 (or affiliate may substitute current resume)

OF-306   (PDF; 92 KB) – Declaration for Federal Employment - an Office of Personnel Management requirement for all employees, contractors, and partners.

IRM-02 (PDF; 74 KB) – Computer User Security Agreement– to be signed by user and NRCS District Conservationist or Supervisor

IRM-03  (PDF; 384 KB) – Request for User Access to ITS Resources - to be completed by NRCS District Conservationist

Computer Security Training

Step 3 - Finger Print Card

For NRCS Employees and Partners call your area office to obtain the SF-87 – Fingerprint Card-RV February 2002 or April 2006.
Remember to complete all applicable boxes.

Have your finger prints taken by your local  Police or Sheriff’s Office.  The cost to the employee for finger printing could vary from 0 to 20 dollars. 

Step 4 - E-Mail e-QIP Invite Form to Cheryl Mitchell at cheryl.mitchell@tn.usda.gov.

Step 5 - Complete all other forms.

Step 6 - New Employee (NRCS or Affiliate) goes to e-QIP website after receiving e-mail from NRCS-NHQ and enters information from SF-85.

Step 7 - Mail complete background investigation package (2 e-QIP certification pages, fingerprint cards, OF-306, OF-612 or resume, IRM-02).
